Follow these steps to successfully complete your petition form.
  • Obtain the light green form that has the title "Petition to the Faculty:  Reinstatement" from       the Admissions office. 
  • Section 1:  Be sure to complete each item in the section box.  Items include name, address, phone numbers, social security number, and major.
  • Section 2:  Check the correct box that states the season in which you plan to return to SPSU.  Check the correct  box that describes the number of dismissals you have had.  Then most importantly, explain any changes you have made to improve your future performance at SPSU.  Do this by proving your side with as much evidence as you are willing to provide.  The more explanation and supporting documentation you are able to submit with this form, the better your chances are for reinstatement to SPSU.  You should attach additional pages/supporting documentation to the back of the form.
  • Section 3:  Read the agreement and sign.
  • Section 4:  Leave this blank.
  • Section 5:  Obtain the signature of your major department head.  This is required!
  • Section 6 (labeled OFFICE USE):  Do nothing.  This is to be completed by the faculty.
